RESIDENT  ADMISSION  BASICS

• Leisure Palms' Manager will personally evaluate applicants to ensure that the facility can meet their needs.   (Free travel to the prospective resident for this purpose is provided in San Diego County and Southern Riverside County if he/she is unable to travel comfortably.)
   

• Law requires all residents to have a TB Test before or soon after admission and a doctor’s statement of health or condition (standard form).
   

• Wheelchair-assisted applicants must be able to get into and out of the wheelchair with the aid of no more than one assistant.
   

• Alzheimer sufferers, with chronic tendency to wander off-premises, may not be accommodated.
   

• People, suffering “Sundowners Syndrome” (panics / outbursts), must be provided with adequate medication to rapidly quell or prevent the onset of such attacks.
   

 
MEDICAL CARE ASSISTANCE

• Medication Administration – Supervision, Assistance, and Monitoring.
   

• Hydration – Supervision, Assistance, and Monitoring.
   

• Transportation to Medical Appointments (local) – Provided by Appointment.
   

• Blood Sugar Monitoring – Supervision of Resident-Administered Tests.
   

• Insulin Injections – Supervision of Resident-Administered Injections.
   

• Confusion / Memory Loss – Supervision Provided.
   

• Incontinence Assistance and Related Supplies – Depuration, Standard Adult Diapers, and Bed Pads provided – if not supplied by Hospice).
   

• Hospice Care –  Cooperative, Integrated Services provided.
   

 
NUTRITION and HYDRATION
Included in Base Price
 

• Three Well-Balanced Meals per day – Prepared in accordance with Residents’ preferences and dietary restrictions.
   

• Residents with Special Restricted Diets – Diabetic or Modified-Texture meals are accommodated.
   

• Snacks – Prepared and offered, periodically, during the day.
   

• Prescribed Hydration - Strictly Monitored.
   

• Assistance to / from Dining Table – Provided.
   

• Hand-Feeding by Caretaker – Provided if Resident is restricted to bed.

 
OTHER SUPPORT SERVICES

• Housekeeping – Provided Daily and As Needed.
   

• Laundry / Bed Linen – Provided on Scheduled and As-Needed Basis.
   

• Wheelchair Assistance – Provided, with One Caretaker’s Assistance.
   

• Routine Ambulatory Assistance – Provided, with One Caretaker’s Assistance.
   

• Bathing, Dressing, and Grooming – Assistance provided, as needed.
   

• Routine Toiletry Supplies – Provided (toothbrush, toothpaste, deodorant, etc.).
   

• Socializing – Encouraged, according to each Resident's Abilities and Wishes.

FLAT RATE  RESIDENTIAL  FEES

 
DAILY RATES for Respite / Recovery (1 – 30 days), provided on a space-available basis.   Daily Rates are pre-paid by weekly or bi-weekly increments.   Vacancy notice: 1-day, or none upon permanent medical evacuation / end of life.   Refunds are issued for unused pre-paid days, following notice.

Semi-Private:  $97/day.                 Private:  $117/day.

MONTH-TO-MONTH (1 or More Months), provided on a space-available basis, pre-paid monthly.   Vacancy notice: 5-days, or none upon permanent medical evacuation or end of life.   Pro-rated refunds are issued for unused time after notice.
